O papel dos municípios na promoção da saúde na Amadora, Lisboa, Mafra e Oeiras

Abstract

This text will present and analyze the role played by Municipalities in relation to the factors which influence the health and well-being of the population, illustrating with case studies from Amadora, Lisbon, Mafra, and Oeiras. Qualitative analysis techniques were used to discuss the information relative to: i) instruments for developing policies which promote health, and ii) the action/interventions with potential for diminishing health inequalities. The results reveal the following aspects: i) the promotion of health within the population is not being presented in explicit terms as a key theme or strategic area amongst the instruments of territorial management; ii) social inclusion is the domain of municipal intervention with the greatest number of activities, and iii) the assessment and monitoring of impacts on health is fundamental for the effective reduction of health inequalities. Within the scope of “the strengthening of community and inter-sectorial action for social inclusion,” an increase was noted in the type and number of interventions occurring over the last seven years. This has been one of the responses from the Municipalities to face the crisis: the need to protect the most vulnerable groups and to diminish social inequalities. In conclusion, the following have thus been identified as catalysts in the promotion of health: the strengthening of the Municipalities’ competences in the area of health, greater multi-level and inter-sectorial articulation, a trans-disciplinary approach, the involvement of other local agents in decision-making processes, and the implementation and monitoring of actions (with impact on health).

Este texto pretende apresentar e analisar o papel dos municípios nos fatores que influenciam a saúde e o bem-estar da população, ilustrando com os casos de estudo da Amadora, Lisboa, Mafra e Oeiras. Foram utilizadas técnicas de análise qualitativa para tratar a informação relativa: i) aos instrumentos para o desenvolvimento de políticas promotoras da saúde e ii) às ações/intervenções com potencial para diminuir as desigualdades em saúde. Os resultados revelam os seguintes aspetos: i) a promoção da saúde da população ainda não é apresentada, de forma explícita, como tema-chave ou área estratégica nos instrumentos de gestão territorial; ii) a inclusão social é o domínio de intervenção municipal com maior número de atividades e iii) a avaliação e monitorização dos impactos em saúde é fundamental para a efetiva redução das desigualdades em saúde. No âmbito do “reforço da ação comunitária e intersectorial para a inclusão social”, verificou-se um aumento crescente no tipo e número de intervenções nos últimos sete anos. Esta tem sido uma das respostas dos municípios à crise: necessidade de proteger os grupos mais vulneráveis e diminuir as desigualdades sociais. Por fim, o reforço das competências dos municípios na área da saúde, a articulação multinível e intersectorial, a transdisciplinaridade, o envolvimento de outros agentes locais nos processos de decisão, implementação e monitorização das ações (com potenciais impactos na saúde) constituem-se como catalisadores na promoção da equidade em saúde.
